Please print and fill out the forms below to bring to your first visit. Along with these forms bring the following items and if applicable a list of medications that you currently take.
- A valid photo ID ( Driver’s license, passport, military ID, etc.)
- Referral slip from your general dentist
- Dental Insurance Card
